A. It covers heavy physical damage, water damage, lost or stolen, etc...basically anything your warranty doesn't cover.

A. Is the phone an LG phone? Most of the time, LG phones are very unreliable, especially a common problem of randomly shutting down. If you currently have insurance, then yes; you will be covered. However, I wouldn't choose LG again.

A. If the insurance is via your carrier, then NO. In nearly all cases, the carriers insurance (if it even applies to an iPhone- many do not) must be for a phone they sold you or is carrier locked to them.
You can buy third party insurance for your device (like homeowners or renters) which will cover the phone.
If you are in the UK, I believe you can buy legally and request an unlock, so look into that.
Also there is some question about the legality and Apple's unwillingness to service iPhones that have been unlocked. If your battery dies, or the screen breaks, 9 out of 10 times they will fix it by swapping out a refurbished phone (not unlocked). Sometimes they will give you grief about the unlocked phone, but usually only if it is user damage. In many states, they must honor the warranty if the defect/repair is unrelated to the software unlock.
They are rather durable, especially with a good case and few plans cover water damage or theft, so you could probably get one and a good case and be fine.
